2027 plan exit lookup

Is My Medicare Advantage Plan Leaving in 2027?

Enter your ZIP code, choose your county, then choose your 2026 plan. You will see the 2027 CMS crosswalk status, the dates that matter, and the other 2027 plans listed in your county.

Have only a plan ID? Decode it first. No sign-in. No SSN. No Medicare number. The lookup runs in your browser. Data version 2026-10-24, last updated October 24, 2026.

Five digits. We use the ZIP only to find your county in your browser.

What the result means

2027 crosswalk status labels
Status What it means What to do
Exiting in 2027 The 2027 crosswalk marks the plan as terminated or non-renewed. Coverage ends December 31, 2026. Choose another plan by December 7, 2026, or use the non-renewal SEP through February 28, 2027.
Continuing in 2027 The plan has a 2027 crosswalk row: renewal, service-area change, or consolidation. Read your Annual Notice of Change and confirm your doctors, drugs, and premium for 2027.
Status not confirmed The plan is not in the 2027 crosswalk, or the row could not be matched. Call the plan or Medicare.gov. The plan may be an employer group, PACE, or cost plan.

What the lookup does not collect

The tool does not ask for your name, address, Social Security number, Medicare number, email, or phone. It does not send your ZIP code to a server. The county data is a static file downloaded once by your browser.

If your plan is not found

Try searching by plan ID (the H-number on your card or letter), by carrier name, or by plan name. If the plan still does not appear, it may be an employer group plan, a PACE plan, a cost plan, or a plan without county-level enrollment in the CMS file. Confirm its 2027 status with the plan or Medicare.gov.

Lookup questions

How does the lookup work?

The tool runs entirely in your browser. It maps your ZIP code to a county using the U.S. Census ZCTA-to-county file, then lists the 2026 Medicare Advantage plans in that county. When you choose a plan, it shows the 2027 status from the CMS Part C&D Plan Crosswalk.

Why is my ZIP code not found?

Phase 1 publishes 30 county pages in ten states. If your ZIP is outside those counties, the tool can still tell you the state, but it does not have a county-level plan list. Use the national page, Medicare Plan Finder, or call 1-800-MEDICARE.

Why is my plan not in the list?

The CMS CPSC files exclude most employer group plans, PACE plans, and cost plans. If your plan is not listed, it may be one of those types, or it may have no county-level enrollment in the file. Confirm the plan status directly with the plan or Medicare.gov.

Does the result tell me what plan to choose?

No. The tool shows status, dates, and other 2027 plans in the county. It does not rank plans or recommend one. Check the premium, network, drug list, and service area with the plan before you enroll.

Important limits

The lookup shows plan-level status from the CMS crosswalk. It does not show 2027 county service areas because CMS has not published them in the CPSC files. A plan marked “Continuing — service area reduced” may still leave your county. Confirm the 2027 service area with the plan.

The lookup does not show 2027 premiums, drug deductibles, networks, or drug lists. Those details are not in the CPSC files. Confirm them with the plan or Medicare Plan Finder before you enroll.
